Our social medical workers can assist you in getting access to the community’s and others’ resources. They can also provide comfort and assistance in the patient’s social and emotional woes along with their families.

• Coordination to community resources (eg. Meals on Wheels, financial aid application, transportation assistance, homemaker assistance)
• Education Regarding Alternative Living Arrangements
• Case Management
• Advance Directives (Power of Attorney for Health Care or Living Will)
• Short-term Therapy
• Crisis Intervention
• Financial Assessment
• Counseling with Life-style Changes
• Teaching Regarding Coping with Loss and Change
• Grief counseling
